What is this Condition?

Spinal cord stimulation is a method used for long-standing (chronic) pain that does not adequately resolve with medication and other treatments. A thin electrode (lead) is placed in the space immediately adjacent to the spinal cord; this electrode is powered by mild electrical signals from a small battery unit placed under the skin. These signals aim to reduce the passage of pain signals to the brain, making the pain more tolerable. Some systems produce a mild tingling sensation, while newer systems may not produce this sensation. Before permanent system placement, a trial period of several days is conducted; if significant pain reduction is achieved, a permanent system is implanted. The goal is not to completely eliminate pain but to significantly reduce it and improve quality of life and mobility.

Symptoms

Patients who have undergone back-spine surgery but continue to experience low back and leg pain

Patients with nerve-origin (neuropathic) chronic pain that is inadequately relieved with medication and physical therapy

Patients with persistent regional pain syndrome in the arm or leg characterized by burning and swelling sensations

Patients experiencing leg pain due to vascular occlusion that cannot be corrected surgically

Patients who first experience significant pain reduction during a short trial period (prerequisite for permanent system)

Patients without contraindications such as active infection, uncontrolled psychiatric illness, or bleeding disorder

Diagnosis Methods

Since this is a treatment method rather than a diagnosis, patient selection and evaluation take precedence over 'diagnosis'. A pain specialist evaluates the type, severity, and intensity of pain and which treatments have been tried previously. Neurosurgery reviews MRI of the spine to determine if there are other problems requiring surgery. Psychiatric evaluation and realistic expectation discussion are performed. The most critical step is the trial period: testing with a temporary electrode for several days; if significant pain reduction is achieved, progression to the permanent system occurs.

Treatment Methods

The application has two phases. In the first phase, usually under local anesthesia and with the patient awake, a thin electrode is placed in the space near the spinal cord under imaging guidance; patient feedback is used to find the correct location. This electrode is tested for several days connected to an external device. If significant pain reduction occurs during the trial, progression to the second phase occurs: a small battery unit is placed under the skin and connected to the electrode. Subsequently, the device is programmed according to the patient's needs. During the first weeks, heavy lifting, bending, and twisting movements should be avoided to prevent electrode displacement.

Permanent system: placement of a small battery unit under the skin and device-specific programming for the patient

Selection of different stimulation types for the patient, including high-frequency systems that do not cause tingling sensation

Regular follow-up after application, updating device settings, and monitoring battery lifespan
