What is this Condition?

Dandy-Walker malformation is a congenital developmental abnormality in the posterior fossa of the brain involving the cerebellum, which controls movement and balance, and the surrounding cerebrospinal fluid-filled spaces. The middle portion of the cerebellum (vermis) is underdeveloped, the fourth ventricle is cystically dilated, and the posterior fossa is enlarged. This abnormality may obstruct cerebrospinal fluid circulation; consequently, most cases present with brain fluid accumulation (hydrocephalus). Symptoms may appear in infancy with rapid head growth, developmental delay, and balance problems; in milder cases, the condition may not be detected until later childhood. The course of the disease is highly variable: in mild isolated cases, development can be largely normal, but when other brain or organ abnormalities are present, the course may be more difficult. Treatment is based on managing fluid accumulation and supporting development.

Symptoms

Rapid head growth in infants (macrocephaly)

Tense or bulging fontanelle, irritability, feeding difficulties, vomiting

Developmental delay (sitting, crawling, walking)

Balance disorder, wide-based gait (due to cerebellar involvement)

Eye movement abnormalities (nystagmus, gaze drift) and seizures

Chronic headache, balance problems, and learning difficulties in milder or late-presenting cases

Diagnosis Methods

This condition is often detected during prenatal ultrasound (at 18-20 weeks); detailed evaluation may include fetal MRI to investigate associated abnormalities. After birth, brain MRI is the primary diagnostic tool; it shows the cerebellum status, dilated spaces, posterior fossa anatomy, and fluid accumulation in detail. In infants with open fontanelles, cranial ultrasound is a useful screening tool; CT may be used in emergency situations or for bone detail. Additionally, developmental assessment tests are performed to monitor the child's development.

Treatment Methods

The foundation of treatment is managing brain fluid accumulation (hydrocephalus) and supporting the child's development. In most cases with fluid accumulation, a shunt system (ventriculoperitoneal shunt) is placed to divert excess fluid from the brain to the abdominal cavity; adjustable pressure systems may be preferred. In selected children, an endoscopic procedure to open fluid pathways (endoscopic third ventriculostomy) may be an option. In large symptomatic posterior fossa cysts, additional interventions are rarely needed. In addition to these interventions, early intervention programs, physical therapy, occupational therapy, speech therapy, and special education when needed are very important; follow-up is a process coordinated by teams of neurosurgery, pediatric neurology, developmental pediatrics, and rehabilitation.
