What is this Condition?

Spasticity is a motor disorder characterized by increased muscle tone, hyperactive velocity-dependent stretch reflexes, and involuntary muscle contractions due to upper motor neuron (pyramidal system) damage. Most commonly seen following cerebral palsy (CP), spinal cord injury (SCI), multiple sclerosis (MS), traumatic brain injury (TBI), and stroke. Cerebral palsy is a group of disorders caused by non-progressive lesions of the developing brain during prenatal, perinatal, or early postnatal period (0-2 years) affecting movement and posture development in childhood. It occurs in 2-3 per 1000 live births worldwide. Etiological factors: prematurity, periventricular leukomalacia (PVL), hypoxic-ischemic encephalopathy (HIE), intraventricular hemorrhage (IVH), infection (meningitis, encephalitis), kernicterus (hyperbilirubinemia), genetic syndromes. CP by spasticity types: spastic diplegia (30-40%, lower limbs), spastic hemiplegia (20-30%, unilateral), spastic tetraplegia/quadriplegia (20%, four limbs), dyskinetic CP (athetoid, dystonic), ataxic CP. Spasticity severely limits activities of daily living, mobility, hygiene, ambulation, and positioning. Chronic spasticity leads to joint contractures, hip dislocation, scoliosis, pain, and sleep disorders. Treatment is multidisciplinary including physical therapy, oral medications (baclofen, tizanidine, dantrolene, benzodiazepines), botulinum toxin (Botox) injections, selective dorsal rhizotomy (SDR), and intrathecal baclofen pump (ITB). ITB pump is the gold standard for drug-resistant, generalized spasticity; the pump implanted in the abdomen delivers continuous baclofen medication into the spinal canal (intrathecal), reducing muscle stiffness by 70-80%. ITB reduces oral baclofen dose 100-fold, minimizing side effects and dramatically improving quality of life. Early intervention, orthotic use, and regular physical therapy prevent contractures and increase functional capacity.

Scissor gait (legs crossing each other)

Joint contractures (elbow, wrist, hip, knee, ankle)

Gait difficulty and toe-walking (equinus)

Loss of hand function (grasping, fine motor skills)

Difficulty with activities of daily living (dressing, hygiene, feeding)

Pain (spasm pain, joint pain, postural pain)

Sleep disturbances (nocturnal spasms, positioning difficulty)

Hip dislocation and scoliosis (secondary in chronic spasticity)

Diagnosis Methods

Spasticity diagnosis is based on clinical examination and neurological assessment. Modified Ashworth Scale (MAS, 0-4) and Tardieu Scale objectively evaluate increased muscle tone. Deep tendon reflexes (DTR) are hyperactive; Babinski reflex is positive (sign of upper motor neuron lesion). Brain MRI is performed early for cerebral palsy diagnosis; structural abnormalities such as PVL, porencephaly, hypoxic-ischemic encephalopathy sequelae, and ventricular dilation are demonstrated. Genetic testing (whole exome sequencing) is considered in atypical cases or with family history. Gait analysis performed in 3D motion laboratory evaluates spasticity patterns, joint angles, and muscle activation. EMG shows muscle activity and spasm frequency. Radiological evaluation: pelvic radiograph for hip dislocation, spine radiograph for scoliosis. Intrathecal baclofen trial dose is applied for ITB pump candidacy; 25-100 mcg baclofen is administered via lumbar puncture with spasticity reduction observed within 4-8 hours. ≥20% tone reduction is a positive response for ITB pump. Differential diagnosis should exclude dystonic CP (static tone, velocity-independent), rigidity (Parkinson-like), lower motor neuron diseases (hypotonia, decreased reflexes).

Treatment Methods

Spasticity treatment requires multidisciplinary and graded approach. First line: Physical therapy and rehabilitation (stretching exercises, ROM - range of motion, strengthening, postural control) and orthotic use (AFO - ankle-foot orthosis, wrist-hand splint). Second line: Oral medications - baclofen (GABA-B agonist, 10-80 mg/day), tizanidine (alpha-2 agonist), dantrolene (calcium release inhibitor), diazepam (benzodiazepine). Disadvantages of oral medications: high dose requirement, central side effects (sedation, weakness, cognitive impairment), inadequate effect in 30-50% of patients. Third line: Botulinum toxin (Botox) injections - for focal spasticity (hamstring, gastrocnemius, adductor) repeated every 3-6 months; provides local effect with no systemic side effects. However, insufficient for generalized spasticity. Surgical options: Selective dorsal rhizotomy (SDR) - in ambulatory children with spastic diplegia (2-6 years), 25-50% of dorsal nerve roots are sectioned; lower limb spasticity is permanently reduced (60-80% success). Intrathecal Baclofen Pump (ITB) - gold standard for drug-resistant, generalized (four limb) spasticity. Programmable pump implanted in abdomen delivers continuous baclofen into spinal canal (intrathecal) reducing spasticity by 70-80%. ITB dose titrated between 50-1500 mcg/day. Pump is refilled every 5-7 years. Advantages: oral dose reduced 100-fold, minimal central side effects, adjustable, reversible. Orthopedic surgery: Tendon lengthenings (Achilles, hamstring, adductor), osteotomies, hip dislocation surgery correct chronic contractures. Combination treatments: ITB + Botox, ITB + orthopedic surgery frequently applied. Early and aggressive treatment prevents contractures and increases functional independence.
