What is this Condition?

Cervical fractures are trauma-related breaks of the cervical vertebrae (C1-C7). Motor vehicle accidents, falls from height, diving into shallow water, and sports injuries are the most common causes. Because the cervical vertebrae surround the most delicate portion of the spinal cord and nerves that affect respiration, fractures in this region require careful management. Fractures can affect the upper (C1-C2) or lower (C3-C7) cervical spine, with each region having specific fracture types. Whether the fracture is stable or unstable and whether there is spinal cord injury determine the treatment approach. In trauma patients, the neck should not be moved, and the head and neck should be immobilized by professional responders until help arrives; early and proper immobilization is crucial for spinal cord protection. Treatment depends on the fracture type and may involve immobilization with a cervical collar or halo vest, or surgery.

Symptoms

Severe neck pain that worsens with movement

Marked tenderness on palpation of the posterior neck

Significant limitation of neck motion and muscle spasm

Weakness, numbness, or tingling in the arms or legs

Breathing difficulty (in upper level injuries)

Bowel or bladder dysfunction (in severe spinal cord involvement)

Diagnosis Methods

In trauma patients, the neck is immobilized with a rigid collar until spinal cord injury is excluded, and neurological examination is performed. Clinical decision rules are used in low-risk patients to reduce unnecessary imaging. The initial imaging modality for fracture detection is cervical CT, which clearly shows the fracture line, fragment displacement, and narrowing of the spinal canal. MRI is important for detecting ligament-disc injuries, spinal cord damage, and injuries not visible on radiographs; it is particularly indicated when neurological findings are present or surgery is planned. CT angiography may be performed if vascular injury is suspected.

Treatment Methods

Treatment is individualized based on whether the fracture is stable or unstable, spinal cord status, and associated injuries. In stable fractures, non-operative management may be preferred; immobilization with a cervical collar or halo vest for typically 6-12 weeks with periodic follow-up imaging to monitor healing. Unstable fractures, those with spinal cord compression, or progressive neurological deficits require surgery. In surgery, vertebrae are fixed with screws and plates (fusion); the approach is selected from anterior or posterior based on fracture location. If spinal cord compression exists, it is relieved. Postoperative care includes intensive care monitoring, early mobilization, and physical therapy.
