What is this Condition?

Spinal stenosis is the narrowing of the spinal canal through which the spinal cord and nerves pass, causing compression of the nerves. It occurs most frequently in the lumbar region; progressive low back and leg pain with walking, numbness in the legs, and decreased walking distance are typical symptoms. Patients often report that they must stop, sit down, or bend forward after walking for a while; these postures temporarily relieve symptoms as the canal widens slightly. The most common cause is age-related changes in the spine such as degeneration and calcification. Symptoms may progress slowly over years and can limit daily activities.

Diagnosis Methods

Evaluation includes physical examination by the physician and MRI; MRI demonstrates the level and degree of canal narrowing. It is important that the narrowing detected on imaging correlates with the patient's actual symptoms, as mild narrowing may not always cause complaints. When necessary, additional evaluation with CT and nerve conduction studies is performed.

Treatment Methods

Treatment is planned stepwise: exercise, pain management, and physical therapy are usually the first approach. Needle injection treatments around the nerve in the canal may be considered in appropriate patients. In patients with significant weakness in the legs or those whose quality of life is severely restricted despite these methods, surgery to widen the canal and reduce nerve compression may be indicated.
