What is this Condition?

Cubital tunnel syndrome results from compression of the ulnar nerve as it passes through the inner aspect of the elbow. This nerve is responsible for the tingling sensation we feel when we hit our elbow on something. When compressed, numbness and tingling occur in the ring finger (4th finger) and little finger (5th finger), as well as on the inner edge of the hand. The compression is generally worsened by prolonged elbow flexion (while sleeping, during phone conversations, or when leaning on the elbow). It is the second most common nerve compression after carpal tunnel syndrome of the wrist. If left untreated, hand muscle atrophy and weakness may develop; therefore, early detection is important.

Symptoms

Numbness and tingling in the ring and little fingers

Numbness and tenderness on the inner edge of the hand (little finger side)

Worsening of symptoms when elbow is bent (sleep, phone use)

Hand weakness: difficulty with gripping, finger opening/closing, and fine motor tasks

Muscle atrophy in advanced cases (especially hollowing of the dorsal hand and little finger base)

Little finger deviation laterally, difficulty holding fine objects between thumb and finger

Diagnosis Methods

Diagnosis begins with physical examination. The physician evaluates Tinel's sign (tingling radiating to fingers upon gentle percussion of the inner elbow), reproduces symptoms by maintaining elbow flexion, and assesses hand muscle strength and atrophy. EMG and nerve conduction studies measure electrical transmission to confirm diagnosis and determine compression severity. Elbow X-rays or ultrasound may be performed to identify bony prominences or masses causing compression.

Treatment Methods

Treatment is planned progressively based on symptom severity. For mild-to-moderate cases, non-surgical approaches are attempted first: night splinting to prevent excessive elbow flexion, avoiding pressure on the elbow, nerve gliding exercises, and pain management. If no improvement occurs within several months or weakness/atrophy develops, surgery is considered. Surgical options include simple decompression (releasing the structures compressing the nerve) or anterior transposition (moving the nerve to the front of the elbow). Method selection depends on individual circumstances; reported success rates are high.

Nerve gliding exercises and pain management as needed

Simple decompression surgery (releasing the structures compressing the nerve)

Anterior transposition (moving nerve to the front of elbow) — in appropriate cases
