What is this Condition?

These tumors develop within the dura (membrane surrounding the spinal cord) but outside the spinal cord itself, around nerve roots. They comprise approximately half of spinal canal tumors and the vast majority are benign. The two most common types are nerve sheath tumors (schwannoma, neurofibroma) and dural tumors (meningioma); in adults these two groups occur with approximately equal frequency. They typically grow slowly and cause symptoms by exerting external pressure on the spinal cord; they do not infiltrate the cord tissue. Therefore, removal to relieve the compression is usually possible. Symptoms may develop insidiously over months or years: pain, weakness, sensory changes, and sometimes urinary-bowel problems. With early diagnosis and appropriate surgery, significant neurological improvement can be achieved in the majority of patients.

Symptoms

Pain at the tumor level; particularly worsening at night and when lying down, not relieved by rest

Pain radiating along a nerve root in an electric or burning quality

Gradually progressive weakness in the arms or legs

Numbness, tingling, or changes in touch or temperature sensation

Difficulty walking, imbalance

In advanced cases, urinary or bowel control problems

Diagnosis Methods

The primary diagnostic method is contrast-enhanced MRI, which shows tumor location, size, relationship to the spinal cord and nerve roots, and provides clues about tumor type. CT is added when detailed visualization of bone structures is needed; angiography may be performed for vascular mapping in highly vascular tumors. Electrodiagnostic tests (such as EMG) can assess neurological function. Definitive diagnosis is established through pathological examination of the resected tumor.

Treatment Methods

Treatment is based on microsurgical removal of the tumor under the microscope with real-time neurophysiological monitoring (MEP, SSEP, EMG) to track neurological function. Since most tumors are benign and located outside the spinal cord, they can usually be largely removed to relieve compression and improve symptoms. For asymptomatic, small, or slowly progressing tumors, or for patients with high surgical risk, serial MRI surveillance at regular intervals may be preferred. In some cases, focused radiation therapy (radiosurgery) is considered for tumors that cannot be completely removed or for recurrent tumors. Post-operative rehabilitation may be planned to support recovery. Treatment is individualized for each patient.
