What is this Condition?

Occipital neuralgia is a recurrent and severe nerve pain that radiates from the back of the head to the vertex, resulting from irritation, compression, or inflammation of the nerves at the back of the neck (greater and lesser occipital nerves). It is a relatively rare type of headache, occurring in a small proportion of patients with chronic headache. It is slightly more common in women and typically presents in middle age. The cause of the pain is often unknown; however, cervical spondylosis and degeneration, neck trauma (such as whiplash from a motor vehicle accident), neck muscle tension, certain cervical joint diseases, or rarely structural problems in the posterior fossa may trigger this pain.

The affected nerve passes between the upper cervical vertebrae and penetrates the occipital muscles to spread to the scalp; these narrow passage points predispose the nerve to compression. Pain is typically unilateral in most cases, begins in the occiput and back of the head, and radiates toward the vertex and temples. Patients describe this pain as resembling an electric shock, stabbing, or burning sensation; attacks may occur paroxysmally or severe pain may be superimposed on constant aching. Extending the neck backward, rotating the head, combing hair, wearing a hat, or resting the head on a pillow may trigger the pain. On examination, pressing over the nerve pathway produces severe pain, and sensory changes may be present in the same area. Treatment follows a stepwise approach: pharmacotherapy, nerve blocks (pain-relieving injections), physical therapy, and surgical or nerve stimulation options are considered in resistant cases.

Symptoms

Severe electric shock-like pain in the occipital region and back of the head

Pain predominantly unilateral (occasionally bilateral)

Radiation of pain toward the vertex and temples

Paroxysmal or continuous burning-type pain

Triggering of pain with neck movements (neck extension, rotation)

Exacerbation of pain with hair combing, hat wearing, and resting head on pillow

Severe pain upon touching the occipital region

Numbness, tingling, or excessive sensitivity in the area

In some patients, photophobia and nausea

Sleep disturbance and impairment of daily activities due to pain

Diagnosis Methods

Diagnosis of occipital neuralgia is based on detailed examination and diagnostic nerve block. For diagnosis, the pain should be localized to the occiput and back of the head along the distribution of the relevant nerve, be electric shock or piercing in quality, produce pain and sensory changes upon pressure over the nerve, and temporarily resolve with analgesic injection (diagnostic block). On examination, pressure over the nerve pathway produces severe pain; excessive sensitivity or numbness to touch may be detected. Cervical range of motion is assessed.

Diagnostic block involves local anesthetic injection (with anti-inflammatory medication if needed) at the nerve pathway; if the pain temporarily resolves within a short time, the diagnosis is confirmed. Cervical and brain imaging (MRI or CT) may be performed to investigate and exclude cervical spondylosis, joint disease, disk pathology, or structural causes in the posterior fossa. In some cases, blood tests are performed to investigate inflammatory causes. Differential diagnosis includes migraine, tension-type headache, cervical radiculopathy, and trigeminal neuralgia; their differentiation is important for appropriate treatment.

Treatment Methods

Occipital neuralgia treatment follows a stepwise approach. First-line treatment is pharmacotherapy: analgesics, muscle relaxants, and specialized neuropathic pain medications (such as gabapentin and pregabalin) as well as certain antidepressant drugs can be used. Second-line treatment consists of nerve blocks: local anesthetic and if necessary anti-inflammatory medication is injected at the pathway of the nerve in the occipital region. This procedure provides temporary relief in a significant proportion of patients; the effect varies from several weeks to several months and can be repeated at regular intervals, but it is not a permanent solution. Physical therapy (cervical stretching exercises, posture training, muscle relaxation, and electrical stimulation methods) is applied as supportive treatment.

Third-line treatment comprises surgical and advanced interventions; these are considered in patients resistant to pharmacotherapy and those with only short-term response to nerve blocks. In nerve decompression surgery, the muscle compartment compressing the nerve is loosened and the nerve is relieved; this method provides long-term pain control in many patients, is performed through a small incision, and hospitalization is brief. Temporary numbness may occur at the operative site. Occipital nerve stimulation can be applied in cases resistant to medications and surgery; fine electrodes are placed along the nerve in the occipital region to modulate nerve activity; this method is adjustable and reversible. Radiofrequency application is another option. If the pain is underlying a specific cause such as cervical spondylosis, joint disease, or a problem in the posterior fossa, treatment is directed toward this underlying cause.
