What is this Condition?

Osteoporotic vertebral fractures occur when vertebrae collapse and cannot withstand pressure due to bone weakening with age (bone loss, osteoporosis). This is the most common type of fracture related to bone loss and is especially prevalent in postmenopausal women. Often, minor trauma such as a slight fall, bending, coughing, or lifting can cause a fracture; in some individuals, it may develop without any obvious cause. It most commonly occurs in the lower thoracic and upper lumbar regions. Each fracture can result in several centimeters of height loss and progressive forward kyphosis over time. The good news is that the vast majority of patients recover with non-surgical methods. Nevertheless, sustaining a fracture is a warning sign for subsequent fractures; therefore, evaluation and treatment of bone loss is very important.

Symptoms

Sudden onset sharp pain in the back or lower back, concentrated at the fracture site

Pain that worsens with sitting, standing up, turning, or bending, relieved by lying down

Progressive height loss noticed over time

Forward kyphosis of the back (especially if multiple fractures are present)

In some individuals, back pain developing without significant trauma

Very rarely, numbness or weakness in the legs (when bone fragments compress a nerve)

Diagnosis Methods

The first step in diagnosis is usually plain radiography; vertebral collapse and deformity are visible here. MRI is performed to determine whether the fracture is new or old, to exclude occult disease (such as tumor or inflammation), and to evaluate neurological signs. CT is used if bone detail is needed. Bone density testing (DEXA) is performed to measure the degree of bone loss, and if necessary, blood tests are performed to investigate underlying causes.

Treatment Methods

The vast majority of patients recover with non-surgical methods, usually within three months. Treatment is based on pain control (analgesics), short-term rest, and early mobilization when possible. A supportive brace may be used in some patients. The most important aspect is treating bone loss to reduce new fractures: calcium and vitamin D supplementation, bone-protective medications, and fall prevention measures. In patients with prolonged pain or severe pain preventing mobility, minimally invasive procedures with special bone cement injection into the vertebra (vertebroplasty, kyphoplasty) may be considered. Treatment is planned individually for each patient.
