What is this Condition?

Thoracic and lumbar fractures are breaks of the vertebrae in the thoracic and lumbar regions of the spine. They most commonly result from falls from height, motor vehicle accidents, or significant trauma, or develop on the basis of bone weakening from osteoporosis. While some fractures heal without compromising spinal stability, others destabilize the spine and create risks such as power loss by compressing the spinal cord or nerves. Therefore, correct determination of fracture type and stability directs treatment. The typical complaint is sudden, severe pain in the back or lower back following the incident; some patients also experience difficulty standing and walking or numbness in the legs.

Diagnosis Methods

In evaluation, CT provides detailed examination of bone structure; MRI shows the status of disc, ligament, and nerve structures. Treatment decision is made by considering both the fracture type and neurological examination findings. In fractures due to osteoporosis, bone quality is also investigated and bone-strengthening therapy may be planned if necessary.

Treatment Methods

In patients whose spine maintains stability and with no nerve involvement, bracing, pain management, and close follow-up may be applied. If spinal stability is compromised, significant deformity is present, or nerves are affected, surgery to stabilize the spine and relieve nerve compression when necessary is considered. In selected fractures due to osteoporosis, injection of bone cement into the fractured vertebra to reduce pain (vertebroplasty or kyphoplasty) may be an option.
