What is this Condition?

Brain herniation is the displacement of brain tissue from its normal compartment to an adjacent compartment or through an opening in the skull due to severely elevated intracranial pressure. This is one of the most serious and life-threatening emergencies in neurosurgery. It can develop in any condition that increases intracranial pressure such as head trauma, intracerebral hemorrhage, stroke, brain tumor, fluid accumulation, meningitis, and cerebral edema. When brain tissue herniates, it compresses vital centers controlling respiratory function, nerves, and blood vessels. The most common type is temporal lobe herniation; the most dangerous type is cerebellar tonsillar herniation causing compression of the respiratory center. Emergency intervention is required within minutes to hours.

Symptoms

One pupil larger than the other, not reacting to light

Rapidly deepening loss of consciousness, inability to arouse

Weakness or paralysis on one side of the body

Irregular breathing, slowing of pulse, and elevated blood pressure (late and severe symptom)

Abnormal posturing in arms and legs

Neck pain and stiffness in cerebellar herniation with risk of respiratory arrest

Diagnosis Methods

Repeated neurological examination is crucial in diagnosis; particularly consciousness level (Glasgow score), pupillary response, and arm-leg strength are frequently assessed. Brain computed tomography is urgent and the first imaging choice of preference; it shows brain displacement, midline shift, and underlying cause (hemorrhage, tumor). Brain MRI provides more detailed evaluation of the brainstem and posterior regions. In critically ill patients, catheters measuring intracranial pressure directly may be used.

Treatment Methods

This is an emergency and minutes matter. Initial management includes securing airway patency, progressing to respiratory support if needed, and administering hypertonic saline or mannitol to rapidly reduce intracranial pressure; head elevation and brief hyperventilation may be supportive measures. Emergency brain CT determines the cause. If there is a mass such as hemorrhage or tumor, emergency surgical evacuation is performed. In widespread edema or high pressure unresponsive to medication, decompressive surgery opening a portion of the skull is performed; if fluid accumulation is present, fluid is drained.

Medications to rapidly reduce intracranial pressure (hypertonic saline or mannitol)

Emergency surgical evacuation of mass such as hemorrhage or tumor

Decompressive surgery opening a portion of the skull in widespread edema
