What is this Condition?

Cerebral edema, or brain swelling, is the accumulation of excess fluid in brain tissue, causing the brain to swell and increase in volume. Since the skull is a closed compartment with fixed volume, as the brain swells, intracranial pressure increases; this can impair brain blood supply and, if untreated promptly, can cause serious damage. It can develop in many conditions such as head trauma, cerebral vascular occlusion (stroke), intracerebral hemorrhage, brain tumor, meningitis, oxygen deprivation, and certain metabolic disorders. Symptoms are usually related to increased intracranial pressure. Severe brain edema is an emergency requiring rapid medical intervention.

Symptoms

Severe headache, worsening in the morning and unresponsive to pain relievers

Nausea and projectile vomiting

Drowsiness, altered consciousness, loss of consciousness in severe cases

Blurred vision, double vision

Seizures (convulsions)

Fontanelle bulging, restlessness, poor feeding in infants

Diagnosis Methods

Diagnosis begins with assessment of consciousness level and neurological examination including pupil examination. Brain CT is rapid and used in initial evaluation; it shows brain swelling, narrowing of fluid spaces, and signs of compression. Brain MRI provides more detailed information and helps differentiate types of edema. In severe patients, direct measurement of intracranial pressure using a special catheter guides treatment. Blood tests are also performed to investigate underlying causes.

Treatment Methods

Treatment addresses both the underlying cause and reduction of intracranial pressure. Medical treatment uses hyperosmolar fluids (mannitol or hypertonic saline) that draw fluid from the brain into the vasculature; corticosteroids are effective in certain conditions like tumors and abscesses but are not appropriate in stroke and trauma. The patient's head is elevated, fever and glucose balance are controlled, and respiratory support and sedation are provided as needed. In resistant cases, decompressive surgery opening part of the skull temporarily, drainage of fluid from the brain, or removal of a mass may be necessary.

Corticosteroids in certain conditions (tumors, abscesses)

Head elevation, fever and glucose control, respiratory support, and sedation

Decompressive surgery, fluid drainage, or mass removal in resistant cases
