What is this Condition?

A depressed fracture is a type of fracture that occurs when a blunt force to the skull causes bone fragments to be compressed inward toward the brain. Traffic accidents, falls, and blunt force injuries are the most common causes. There are two main types: closed fractures where the skin and brain membranes remain intact, and open fractures where the skin and membranes are torn, exposing the bone to the external environment. Open fractures require more careful management due to higher risk of infection from microorganisms. The depressed bone fragment may damage underlying brain tissue or cause bleeding. Possible complications include seizures, infection, and cerebrospinal fluid leakage from the nose or ears. Treatment is planned based on the depth of depression, integrity of the membranes, and any accompanying injuries.

Symptoms

Swelling, bleeding, and a palpable depression at the site of impact

Clear fluid (cerebrospinal fluid) leaking from the nose or ears

Headache and vomiting

Alterations in consciousness (if accompanying brain injury is present)

Weakness on one side of the body, speech or vision disturbances

Seizures

Diagnosis Methods

The primary and most important examination is brain computed tomography (CT); it reveals the fracture line, the extent of bone depression, and any accompanying hemorrhage or brain injury. Three-dimensional CT imaging is useful in clarifying fracture shape and surrounding structures for surgical planning. When fluid leaks from the nose or ears, a special laboratory test can be performed to determine if it is cerebrospinal fluid. In open fractures, blood tests and cultures are obtained to assess for infection; electroencephalography (EEG) may be performed if seizures are suspected.

Treatment Methods

Treatment is determined by the type of fracture and any accompanying injuries. In superficial, closed, and minimally depressed fractures with intact brain membranes and no additional damage, conservative management with observation may be possible; this includes pain control, seizure prophylactic medication if needed, and follow-up CT imaging. Surgery is required for significant depression, open fractures, membrane tears, mass effect from bleeding, or obvious deformity. During surgery, depressed bone fragments are elevated and secured in proper position; membrane tears are repaired if present. In open fractures, the wound is cleaned, cerebrospinal fluid leakage is prevented, and antibiotics are administered to reduce infection risk. Accompanying hemorrhage can be managed during the same procedure.
