What is this Condition?

Dissection is a small tear between the wall layers of large arteries (carotid artery or vertebral artery) that travel from the neck to the brain, with blood seeping into the artery wall from this tear and accumulating there. The accumulated blood can narrow the inside of the artery or even block it; this disrupts blood flow to the brain and increases stroke risk. Dissection is responsible for a significant portion of strokes seen in young and middle-aged individuals. While some develop following neck trauma, sudden neck movements, or neck manipulation, others develop spontaneously without a clear cause, sometimes on the basis of an underlying condition that weakens the artery wall. It typically presents with sudden-onset neck-head pain accompanied by neurological symptoms. The good news is that the vast majority of patients recover spontaneously within a few months; during this period, treatments that prevent blood clot formation reduce stroke risk.

Symptoms

Sudden-onset, unilateral neck pain (may radiate to the anterior neck/jaw or occiput)

Severe, unusual headache

Drooping eyelid and pupil constriction (in carotid artery type)

Pulsatile tinnitus in the ear

Stroke/transient ischemic attack symptoms: weakness on one side, speech disturbance, sensory loss

Vertigo, diplopia, balance disturbance, and dysphagia (in vertebral artery type)

Diagnosis Methods

Initial evaluation uses CT angiography or MR angiography to visualize vessels; these are rapid and non-invasive methods that demonstrate artery narrowing, blood accumulation within the wall, and other typical findings with high accuracy. Diffusion MR is performed to assess whether stroke has occurred in the brain. If needed, particularly when endovascular treatment is planned, digital subtraction angiography (DSA) can be performed. Neck vessel Doppler can be used for follow-up but may not be sufficient alone.

Treatment Methods

The goal of treatment is to prevent thrombotic stroke and preserve cerebral blood flow. In most patients, the primary treatment is medication that prevents clot formation: anticoagulation or antiplatelet agents (such as aspirin); which is preferred depends on the patient's condition and is typically continued for several months. It is important to avoid excessive elevation or decrease in blood pressure. In selected patients with progressive narrowing despite medical therapy, recurrent symptoms, or insufficient blood flow, endovascular stent/balloon intervention may be considered. Surgery (such as bypass) is considered in very rare circumstances. If brain hemorrhage is present, blood pressure control and specific interventions for the situation may be required. In most patients, the artery recovers spontaneously within a few months; follow-up imaging is used for monitoring.
