What is this Condition?

Encephalitis is inflammation of brain tissue. It is most commonly caused by viruses; it can also develop when the immune system attacks its own brain (autoimmune). One of the most frequent and most dangerous viral types is encephalitis caused by herpes simplex virus; if untreated, it can cause serious consequences, but early initiation of antiviral medication significantly reduces the risk. Encephalitis differs from meningitis in that it affects brain tissue rather than brain membranes, so altered consciousness, behavioral disturbances, seizures, and neurological deficits are prominent features. Sometimes both conditions occur together. In case of suspicion, starting treatment without waiting for diagnostic tests is of vital importance.

Symptoms

Fever and severe headache

Altered consciousness: drowsiness, confusion, and in advanced cases, loss of consciousness

Seizures

Behavioral and personality changes, irritability, hallucinations

Memory problems, speech disturbances

Weakness on one side of the body or balance disturbance

Diagnosis Methods

Diagnosis involves examination of cerebrospinal fluid obtained by lumbar puncture; genetic (PCR) testing to detect viruses, particularly herpes simplex virus, is the most reliable method in herpes simplex encephalitis. Brain MRI is highly valuable; herpes simplex encephalitis typically shows inflammation in the temporal region. Brain electroencephalography (EEG) and blood tests support diagnosis. MRI and tests may be normal in the first few days; this does not exclude encephalitis, and if clinical suspicion persists, treatment is continued.

Treatment Methods

When herpes simplex encephalitis is suspected, intravenous antiviral medication (acyclovir) is started immediately without waiting for diagnostic tests; because early treatment significantly reduces the risk. Treatment typically lasts 14-21 days. Seizure-prevention medications are given for seizures, and if cerebral edema develops, pressure-reducing treatment is applied; respiratory support is provided if needed. In autoimmune encephalitis, immunosuppressive therapies such as corticosteroids, intravenous immunoglobulin (IVIG), or plasma exchange (plasmapheresis) are used; any triggering tumor is treated. Rehabilitation is important during the recovery process.
