What is this Condition?

Hepatic encephalopathy is a cerebral dysfunction occurring when the liver is unable to perform its function adequately. Normally, the liver clears toxic substances such as ammonia from the intestine. In liver failure (for example, cirrhosis), these substances accumulate in the blood and reach the brain, negatively affecting thought, attention, and consciousness. The presentation can range from very mild (attention deficit detectable only by testing) to marked mental confusion, deep sleep, and even coma. Most often a precipitating factor (infection, constipation, gastrointestinal bleeding, fluid-electrolyte imbalance, or certain medications) triggers an episode. When the precipitating factor is corrected and appropriate treatment is given, episodes are usually reversible.

Symptoms

Difficulty concentrating, forgetfulness, poor attention

Disrupted day-night rhythm, daytime somnolence, nocturnal wakefulness

Disorientation to time and place, inappropriate behavior, personality changes

Asterixis: inability to maintain posture when arms are extended forward and wrists dorsiflexed, causing a flapping tremor-like movement

Speech slowing and deterioration, slowness in movements

In severe cases, deep somnolence, unresponsiveness, and coma

Diagnosis Methods

Diagnosis is usually based on clinical examination: in a patient with known liver disease, if typical mental and neurological findings are present and other causes are excluded, diagnosis is established. Blood ammonia levels can be checked, but elevated or normal levels do not independently establish or exclude diagnosis. Precipitating factors are investigated by searching for infection, bleeding, and electrolyte abnormalities. In unclear or very mild cases, tests measuring attention-reaction time are used; EEG or brain imaging may be performed as needed.

Treatment Methods

The first step of treatment is to identify and correct the precipitating factor (for example, treating infection, relieving constipation, stopping bleeding). To reduce toxic substance formation in the intestine, lactulose syrup is given; the aim is to achieve two to three soft bowel movements daily. This is often combined with rifaximin, a non-absorbed antibiotic, which helps reduce episode recurrence. Protein restriction, previously recommended, is no longer advised; adequate protein intake is important to prevent muscle wasting. Severe cases may require hospitalization and intensive care.

Identification and treatment of the precipitating factor (infection, constipation, bleeding, etc.)

Lactulose syrup (reduces intestinal absorption of toxic substances, promotes regular bowel movements)

Rifaximin (non-absorbed antibiotic; added to lactulose to reduce recurrences)

Adequate and balanced nutrition; evaluation for liver transplantation in advanced liver failure
