What is this Condition?

Lyme disease is a bacterial infection transmitted to humans through an infected tick bite. The tick usually transmits the microorganism to the body after remaining attached to the skin for several days. The initial symptom is often a red rash at the bite site that gradually expands in a ring shape with a clear center (target appearance). The disease is not limited to the skin and may progressively affect the nervous system, heart, and joints. Nervous system involvement (neuroborreliosis) most commonly manifests as facial palsy, meningitis-like headache and neck stiffness, or radicular pain. When appropriate antibiotics are started early, the vast majority of patients fully recover.

Symptoms

Red rash at the bite site with clearing center, expanding in a ring pattern (usually painless and non-pruritic)

Flu-like symptoms: fever, malaise, headache, muscle and joint pain

Drooping of one side of the face (sometimes both sides), difficulty closing the eye (facial palsy)

Severe pain radiating along a dermatome, worsening at night

Severe headache and neck stiffness (signs of meningitis)

Recurrent swelling and pain in joints (particularly the knee); rarely cardiac symptoms such as palpitations and syncope

Diagnosis Methods

If the typical ring-shaped rash is seen in the context of tick exposure, diagnosis is usually made clinically and blood testing is not required. In other cases, two-tier blood testing (screening test followed by confirmatory test) is performed. Early-stage blood tests may still be negative. If nervous system involvement is suspected, lumbar puncture may be performed to examine cerebrospinal fluid; brain or spinal cord MRI is obtained if needed.

Treatment Methods

Treatment is based on antibiotics and is highly effective when started early. In the early, skin-limited stage, oral antibiotics (such as doxycycline or amoxicillin in pregnant women and young children) are usually sufficient. Intravenous antibiotics (ceftriaxone) are preferred for nervous system involvement, though oral treatment may be used in some mild cases. Treatment duration is typically two to four weeks. Facial palsy, meningitis, and joint involvement show marked improvement within weeks with appropriate treatment.

Oral antibiotics (for early-stage skin involvement)

Intravenous antibiotics (in nervous system or severe cardiac involvement)

Supportive care and follow-up for symptoms such as facial palsy and meningitis

Prevention: protective clothing in nature, insect repellent use, and early and proper tick removal
