What is this Condition?

Non-functional pituitary adenoma is a benign tumor that develops in the pituitary gland, the hormonal control center located at the base of the brain. Unlike some other pituitary tumors, this tumor does not produce excessive hormones; therefore, it may grow for a long time without causing symptoms. It is usually discovered when it grows large enough to compress surrounding structures, particularly the optic nerves. The most common complaint is related to vision; narrowing of peripheral vision is a typical finding. The growing tumor can suppress the pituitary's normal hormone production, leading to problems such as fatigue and hormone deficiency. In most patients, diagnosis is made when the tumor reaches a significant size, and treatment is usually surgical.

Symptoms

Vision disturbance, particularly narrowing of peripheral vision (bilateral vision loss)

Persistent headache (usually in the forehead or behind the eyes)

Fatigue, tiredness, and general debility related to hormone deficiency

Irregular menstruation in women, loss of libido in men (hormonal imbalance)

Intolerance to cold, weight changes, loss of appetite

Rarely, sudden severe headache and vision loss (tumor hemorrhage)

Diagnosis Methods

The first step in diagnosis is contrast-enhanced pituitary MRI; the size of the tumor and whether it compresses the optic nerve and surrounding structures are evaluated here. Visual field testing measures the loss of peripheral vision. Additionally, blood tests check whether there is any deficiency in the pituitary's hormone production. The definitive type is determined by examining the tissue sample obtained during surgery.

Treatment Methods

The first line of treatment is usually surgery; the tumor is removed through a closed (endoscopic) approach via the nose. This method helps reduce compression on the optic nerve and promotes vision improvement. If the tumor cannot be completely removed or recurs over time, radiation therapy (gamma knife or radiotherapy) may be added. If pituitary hormone deficiency develops, missing hormones are supplied externally in pill or injection form to restore balance. Since this tumor does not produce excessive hormones, medical shrinkage is usually not possible.

Tumor removal through closed (endoscopic) surgery via the nose (first-line treatment)

Radiation therapy (gamma knife or radiotherapy) if tumor cannot be completely removed or recurs

Administration of missing pituitary hormones externally if hormone deficiency develops (hormone replacement)

Regular MRI surveillance instead of surgery in small, asymptomatic tumors
