What is this Condition?

Obstructive hydrocephalus is the accumulation of fluid in the brain ventricles (cerebral cavities) due to blockage in the pathway where cerebrospinal fluid (CSF) circulates, which protects the brain and spinal cord. Normally, this fluid is continuously produced, circulates along the channels within the brain, and is absorbed. When an obstacle in the pathway (such as a tumor, cyst, hemorrhage, or congenital narrowing) blocks this circulation, fluid accumulates and expands the brain cavities, increasing pressure. This can result in headache, nausea, vision changes, and in progressive cases, altered consciousness. If the obstruction develops suddenly, pressure can rise rapidly, which may require emergency intervention; delay increases the risk of permanent damage. The goal of treatment is to open the blocked pathway or provide a new flow route for the fluid, thereby safely reducing pressure. If there is an underlying cause (tumor, cyst, etc.), targeted treatment is also planned. With early diagnosis and appropriate intervention, symptoms improve in most patients.

Symptoms

Headache, particularly worsening in the morning

Nausea and vomiting

Blurred vision or diplopia

Balance disturbance and drowsiness

Altered consciousness in advanced cases

Diagnosis Methods

Diagnosis uses computed tomography (CT) or magnetic resonance imaging (MRI) of the brain; these images show how much the brain ventricles have enlarged and where the obstruction is located. A tumor, cyst, hemorrhage, or congenital narrowing causing the obstruction is searched for. Simultaneously, fundoscopic examination and neurological status assessment are performed to monitor the effects of increased pressure. Thus, both the location and cause of the obstruction are clarified for treatment planning.

Treatment Methods

Treatment is planned according to the cause and location of the obstruction. In some patients, an endoscopic procedure (ETV) that creates a new pathway for fluid within the brain is performed. In other cases, a drainage system (shunt) that directs fluid to another body compartment is implanted. If there is a lesion such as a tumor or cyst causing the obstruction, targeted treatment is also performed. When pressure rises suddenly and dangerously, temporary drainage that empties fluid externally may be needed until permanent resolution is achieved. The common goal of all these methods is to safely restore cerebrospinal fluid circulation.
