What is this Condition?

Osmotic demyelination syndrome is myelin damage surrounding brain cells that occurs as a result of overly rapid correction of chronically low blood sodium levels (hyponatremia). The pons region in the brainstem is most commonly affected. The brain adapts over time to low sodium; if sodium is corrected suddenly and rapidly, brain cells cannot keep pace with this change and damage develops. Typically it has a biphasic course: initially the patient improves as sodium is corrected, then new neurological findings appear several days later. This presentation can include limb weakness, swallowing and speech difficulty, and altered consciousness. The most important point is prevention: low sodium should be corrected slowly and in a controlled manner.

Symptoms

Temporary improvement initially as sodium is corrected, followed by new symptoms onset within 2-6 days

Limb weakness, progressively affecting all four limbs with rigidity

Swallowing difficulty, speech deterioration and unintelligibility

Diplopia, restricted eye movements

Mental confusion, lethargy, in severe cases profound somnolence

In the most severe form, locked-in syndrome: consciousness preserved but body paralyzed (communication only through eye movements)

Diagnosis Methods

History is highly determinative in diagnosis: chronic low sodium followed by rapid correction and biphasic course is typical. Brain MRI confirms diagnosis; characteristic appearance in the pons region is noted. However, MRI may be normal in the first one to two days, so if clinical suspicion persists, it is repeated after several days. Changes in sodium level are closely monitored and other electrolyte abnormalities are investigated.

Treatment Methods

There is no proven definitive treatment for this condition; therefore, the most effective approach is prevention. Low sodium, particularly in high-risk individuals, is corrected slowly and in a controlled manner to remain below established daily limits; sodium is measured at frequent intervals. If sodium has risen faster than intended, the physician may take measures to re-lower sodium somewhat. When the condition develops, treatment is supportive: respiratory safety, tube feeding, thrombosis prevention, treatment of infections, and intensive physical therapy-rehabilitation are applied.

Prevention: slow, controlled correction of low sodium and frequent monitoring (most important step)

Re-balancing under physician control if sodium rises faster than necessary

Supportive care: respiratory safety, safe nutrition, thrombosis and infection prevention

Physical therapy, speech-swallowing therapy, and long-term rehabilitation
