What is this Condition?

Pineal region tumors are rare tumors that originate from the pineal gland (epiphysis) and surrounding tissues located deep in the exact center of the brain. The pineal gland secretes melatonin, the hormone that regulates the sleep-wake cycle. This region can contain many different types of tumors; some are benign and slow-growing, while others may progress more rapidly. Therefore, the treatment plan is determined based on the tumor type and patient age. The tumor can obstruct cerebrospinal fluid flow, leading to fluid accumulation (hydrocephalus), which causes headache and nausea. Because this region is adjacent to important blood vessels and brain structures, evaluation and treatment require an experienced team.

Symptoms

Difficulty with upward gaze and eye movement disorders

Headache, nausea, and vomiting related to fluid accumulation (hydrocephalus), worsening in the morning

Unsteady gait and coordination disorder

Double vision and vision problems

Hormonal changes such as early puberty in children

Fatigue, changes in attention and memory

Diagnosis Methods

Contrast-enhanced brain MRI is the fundamental diagnostic method; the location, type of tumor, and fluid accumulation are assessed. Spinal cord MRI may be performed to determine if the disease has spread to the spinal cord. Some tumor types elevate specific marker substances in the blood and cerebrospinal fluid (tumor markers), so these tests are requested. In most cases, a small tissue sample (biopsy) is necessary for definitive diagnosis.

Treatment Methods

Treatment varies significantly based on tumor type. In some types, drug therapy (chemotherapy) and radiation therapy are sufficient after tissue sampling, while in other types, surgical resection of the tumor is the primary focus. If fluid accumulation (hydrocephalus) is present, an endoscopic procedure or drainage procedure may be needed for relief. Treatment is a multidisciplinary effort planned jointly by neurosurgery, oncology, and radiation oncology specialists.
