What is this Condition?

Pott's disease is the most common form of spinal tuberculosis, caused by Mycobacterium tuberculosis affecting spinal bones and intervertebral discs. Usually develops through hematogenous spread from pulmonary tuberculosis; most commonly affects the thoracic and lumbar spine. The disease progresses slowly: infection starts from the vertebral body, spreads to the disc and adjacent vertebra. If untreated, vertebral collapse and forward kyphosis (gibbus deformity) may develop, along with inflammatory accumulations called cold abscess around the spine and nerve compression. Due to insidious onset, diagnosis is often delayed. The cornerstone of treatment is prolonged anti-tuberculous medication; surgery is sometimes necessary.

Symptoms

Persistent and progressive thoracic or lumbar pain (may continue at night)

Forward kyphosis of the thoracic spine, spinal deformity (gibbus)

General tuberculosis symptoms such as malaise, loss of appetite, weight loss, night sweats, and low-grade fever

Leg weakness, numbness; walking difficulty in progressive cases

Loss of urinary and bowel control (if nerve compression progresses)

Swelling around the spine or in the groin (cold abscess)

Diagnosis Methods

Spinal MRI is the most valuable diagnostic method; it shows vertebral and disc involvement, paravertebral abscesses, and nerve compression in early stages. X-ray and CT assess bone destruction and collapse. For definitive diagnosis, demonstration of tuberculosis bacteria in a sample (biopsy) from the affected area is important. Additionally, blood tests and pulmonary evaluation are performed, as the disease often originates from the lungs.

Treatment Methods

The cornerstone of treatment is the combined and prolonged use of multiple anti-tuberculous drugs (usually 12-18 months); regular and complete adherence to medications is critical for recovery. Most patients recover with medication therapy alone. Surgery is indicated for significant nerve compression, progressive neurological deficit, severe vertebral collapse with spinal instability and deformity, or large abscess not responding to medication. Surgery relieves compression and, if necessary, stabilizes the spine (stabilization). Physical therapy and nutritional support are also important adjuncts.

Long-term anti-tuberculous drug therapy: Combined use of multiple drugs regularly for 12-18 months is the foundation of treatment.

Surgical decompression: Inflammatory tissue, necrotic material, or collapsed bone compressing nerves is removed.

Spinal stabilization (stabilization/fusion): In severe collapse or deformity, the spine is supported with screw-rod systems and grafts.

Supportive care: Abscess drainage, physical therapy, nutritional support, and regular follow-up complete the treatment.
