What is this Condition?

Pseudotumor cerebri is a condition in which intracranial pressure increases without a brain tumor, fluid accumulation (hydrocephalus), or other structural problem. The term 'pseudotumor' meaning 'false tumor' in its name comes from the fact that symptoms produce headaches and visual disturbances similar to brain tumors. It is most commonly seen in obese young women of reproductive age. Symptoms typically include prolonged headaches, blurred vision, and swelling of the optic nerve at the back of the eye (papilledema). Although the exact cause is not fully understood, obesity is the strongest risk factor. With early diagnosis and treatment, symptoms improve in most patients; the main goal is to prevent permanent vision loss.

Symptoms

Prolonged headaches, usually in the morning and worsening with forward bending

Pulsatile tinnitus synchronized with heartbeat

Transient visual obscurations lasting seconds, especially with position changes

Visual field constriction, blurred vision

Double vision

Nausea and vomiting

Diagnosis Methods

Diagnosis involves brain MRI and imaging showing brain vasculature (MR venography) to exclude tumors, fluid accumulation, or vascular occlusion. Subsequently, lumbar puncture is performed to measure cerebrospinal fluid pressure, which is elevated, although the fluid composition is normal. Ophthalmologic examination of the optic disc, visual field testing, and imaging measuring optic nerve thickness are performed regularly by an eye specialist.

Treatment Methods

Treatment aims to reduce intracranial pressure, resolve optic nerve swelling, and prevent permanent vision loss. Weight loss is the most effective step; losing five to ten percent of body weight can significantly reduce symptoms. Additionally, medications such as acetazolamide, which has a diuretic effect, reduce pressure by decreasing fluid production; other medications may be added if needed. If vision continues to deteriorate despite medication, surgery to relieve compression around the optic nerve, shunt placement, or in selected cases, endovascular procedures may be considered.

Medications reducing intracranial fluid production (acetazolamide and additional medications if necessary)

Surgical procedure to relieve optic nerve compression if vision is at risk

Shunt placement in resistant cases or endovascular stent in selected patients
