What is this Condition?

Shunt infection occurs when a shunt tube system, placed to drain excess cerebrospinal fluid (CSF) from the brain to another body cavity, becomes infected with microorganisms. It is one of the clinically significant complications of shunt surgery and occurs most often within the first six months, particularly within the first two months. Microorganisms, usually bacteria naturally present on our skin, typically become inoculated onto the tube system during surgery and form a biofilm on the tube surface that protects them from antibiotics. Therefore, infection may not resolve with antibiotics alone; often the infected shunt must be removed. Success rate is high if detected early.

Symptoms

Fever, chills (fever may occasionally be absent)

Severe headache, nausea and vomiting

In infants: bulging of the fontanelle, irritability, inability to feed

Neck stiffness, photophobia

Redness, swelling, tenderness, or drainage along the subcutaneous shunt tract

In patients with abdominal catheters: abdominal pain and distension

Diagnosis Methods

The most reliable diagnostic method is cerebrospinal fluid examination. Fluid is typically obtained through a needle puncture from the subcutaneous shunt reservoir and cultured to identify the causative microorganism. Elevated inflammatory cells, elevated protein, and decreased glucose in the fluid support infection. Additionally, blood tests, brain CT/MRI if necessary, and X-ray imaging showing the entire shunt system are used.

Treatment Methods

The accepted standard treatment consists of complete removal of all infected shunt components, temporary external drainage of brain fluid (external ventricular drainage), and intravenous antibiotic therapy. Antibiotic duration varies with the organism and typically lasts 10-21 days; it may be longer for fungal infections. Once sterile cultures confirm the fluid is free of microorganisms over consecutive samplings, a new shunt is usually placed within 7-14 days. Medication alone is generally insufficient because bacteria are protected by the biofilm on the tube surface.

Complete removal of the infected shunt and temporary external drainage of brain fluid

Intravenous antibiotic therapy (10-21 days depending on organism, longer for fungal infections)

Placement of a new shunt after confirmation that the fluid is microorganism-free

Direct injection of antibiotics into the tube in certain cases
