What is this Condition?

Peripheral nerve injury is nerve damage caused by trauma (laceration, blunt, gunshot, avulsion), surgical complication, compression (compartment syndrome, tourniquet), traction (brachial plexus) or iatrogenic causes. The peripheral nervous system consists of nerves conveying motor, sensory and autonomic functions; post-injury weakness (motor deficit), numbness/paresthesia (sensory deficit) and vasomotor changes (autonomic dysfunction) develop. Nerve injury severity is classified by Seddon classification (3 degrees) or Sunderland classification (5 degrees). Seddon classification: (1) Neurapraxia - myelin sheath damage, axonal continuity preserved, complete recovery in 6-12 weeks; (2) Axonotmesis - axon severed but endoneurium (nerve sheath) intact, Wallerian degeneration occurs, regeneration slow (1 mm/day = 1 inch/month), intermediate prognosis; (3) Neurotmesis - complete nerve severance, all layers including endoneurium damaged, no spontaneous recovery, surgery required. Sunderland classification: Grade I (neurapraxia), Grade II (axon severed, endoneurium intact), Grade III (axon + endoneurium severed, perineurium intact), Grade IV (axon + endoneurium + perineurium severed, epineurium intact - neuroma-in-continuity), Grade V (complete transection - neurotmesis). Prevalence of nerve injury in trauma patients is 1-3%; upper extremity (median, ulnar, radial nerves) affected more frequently than lower extremity. Most common mechanism is laceration (30-40%), gunshot (20-30%) and blunt trauma (15-20%). Brachial plexus injury (avulsion) seen in motorcycle accidents. Diagnosis made by clinical examination (motor/sensory/autonomic tests), electrophysiological evaluation (EMG/NCS - detects Wallerian degeneration 2-3 weeks post-injury, fibrillation potentials), ultrasonography (high-resolution US - nerve continuity, neuroma, scar tissue) and MR neurography (plexus avulsion, nerve anatomy). Treatment approach determined by injury type: neurapraxia/Grade I - conservative follow-up (complete recovery 6-12 weeks); axonotmesis/Grade II - conservative follow-up + physical therapy (regeneration 3-6 months); neurotmesis/Grade III-V - surgical repair. Surgical techniques: primary repair (end-to-end repair - sutured without tension, success increases if within 72 hours), nerve grafting (sural, medial antebrachial cutaneous nerve graft - for tension-free closure), nerve transfer (transfer of adjacent nerve fascicle to accelerate motor regeneration in proximal lesions) and nerve conduit (artificial nerve guide - for short defects <3 cm). Post-operative physical therapy, splinting and electrical stimulation applied. Prognosis depends on injury type, patient age, delay duration and nerve type; motor regeneration slower than sensory, prognosis poor in proximal lesions. Early diagnosis and surgery (within 3-6 months) improve functional outcomes.

Symptoms

Motor deficit (weakness or paralysis - in muscles innervated by affected nerve)

Sensory deficit (numbness, anesthesia, hypoesthesia - in nerve's sensory distribution)

Paresthesia (tingling, burning, pins-and-needles sensation)

Neuropathic pain (sharp, electric shock-like, spontaneous or provoked)

Neuroma pain (scar tissue developing at nerve end - positive Tinel sign, percussion-induced pain)

Muscle atrophy (muscle wasting due to denervation - develops within 3-6 months)

Autonomic dysfunction (decreased/increased sweating, skin color changes, vasomotor instability)

Complex regional pain syndrome (CRPS - severe pain, edema, trophic changes)

Claw hand deformity (ulnar nerve injury) or wrist drop (radial nerve injury)

Hoffmann-Tinel sign (distal paresthesia with percussion at injury site - sign of regeneration)

Diagnosis Methods

Peripheral nerve injury diagnosed by clinical examination and electrophysiological testing. Physical examination assesses motor function (muscle strength - Medical Research Council (MRC) scale: 0 = no contraction, 5 = normal strength), sensory function (two-point discrimination, monofilament testing, pinprick), autonomic function (sweating, skin temperature) and reflexes. Tinel test (distal paresthesia with percussion at injury site - positive if regeneration ongoing) and neuroma presence assessed. Electromyography and nerve conduction studies (EMG/NCS) performed 2-3 weeks post-injury (may be falsely negative early); evaluates Wallerian degeneration (degeneration of distal nerve segment), fibrillation potentials (denervation), compound muscle action potential (CMAP) amplitude loss and conduction block. EMG/NCS determines injury degree (neurapraxia vs axonotmesis vs neurotmesis) and prognostic estimation. High-resolution ultrasonography (US) shows nerve continuity, neuroma, scar tissue and nerve thickness; dynamic assessment (nerve mobility) and real-time imaging advantageous. Magnetic resonance neurography (MRN) evaluates brachial plexus avulsion, nerve anatomy and surrounding structures (vessels, muscles). Computed tomography (CT) assesses associated bone fractures and bony impingement. Video recording or photograph of injury site (if available) aids understanding of mechanism.

Treatment Methods

Peripheral nerve injury treatment determined by injury type (Seddon/Sunderland classification) and duration. Neurapraxia (Seddon Grade I / Sunderland Grade I) completely recovers with conservative treatment in 6-12 weeks; physical therapy (passive joint range of motion, electrical stimulation), splinting (prevent contractures) and analgesic therapy (neuropathic pain managed with gabapentin, pregabalin) applied. Axonotmesis (Seddon Grade II / Sunderland Grade II) regenerates at 1 mm/day (1 inch/month) rate; conservative follow-up and physical therapy for 3-6 months may restore function, but if regeneration does not occur or is inadequate (reassess at 3-6 months with EMG/NCS), surgical indication arises. Neurotmesis (Seddon Grade III / Sunderland Grade III-V) no spontaneous recovery, surgery required. Surgical timing: laceration injuries warrant primary repair (within 72 hours - clean laceration, no contamination), delayed primary repair (7-10 days - contaminated wound), or late repair (3 weeks - 6 months - gunshot, avulsion, inadequate assessment). Surgical techniques: (1) Primary repair (end-to-end epineurial or fascicular neurorrhaphy - tension-free with 8-0 or 9-0 non-absorbable sutures), (2) Nerve grafting (autograft for tension-free closure - sural nerve, medial antebrachial cutaneous nerve), (3) Nerve transfer (accelerate motor regeneration in proximal lesions - e.g. Oberlin procedure, transfer ulnar nerve fascicle to the musculocutaneous nerve (biceps motor branch)), (4) Nerve conduit (artificial nerve guide - for short defects <3 cm, collagen or polymer tube), (5) Neurolysis (removal of scar tissue around nerve - neuroma-in-continuity). Post-operative immobilization (3 weeks), physical therapy (passive/active ROM, strengthening), electrical stimulation (delay muscle atrophy) and splinting (prevent contractures) applied. Prognostic factors: age (younger patients better outcome), injury type (laceration > blunt > avulsion), delay duration (early surgery <6 months better), proximity (distal lesions better prognosis), nerve type (pure motor/sensory > mixed). Motor regeneration success rates: primary repair 60-80%, nerve grafting 50-70%, nerve transfer 60-90%. Early diagnosis and surgery significantly improve functional outcomes.
