What is this Condition?

Slit ventricle syndrome is a complication that can develop over years in patients with hydrocephalus treated with a shunt. When the shunt drains excess cerebrospinal fluid from the brain for a prolonged period, the fluid-filled spaces (ventricles) within the brain become extremely small and their walls become stiff and lose elasticity. This can result in two types of symptoms: either the catheter tip adheres to the wall and intermittently occludes, causing sudden severe headache crises, or excessive drainage causes pressure to drop significantly, resulting in positional headaches. It occurs in some shunt-dependent patients, particularly in those with early shunt placement in infancy and those dependent on shunts for long periods. Treatment is planned individually.

Symptoms

Sudden-onset severe headache crises lasting minutes to hours

Nausea, vomiting, and photophobia accompanying headache episodes

Positional headaches worsening when upright or sitting, relieved when lying down (low pressure type)

Pain triggered by coughing, straining, or position changes

Decline in attention, memory, and school performance, fatigue

Restlessness and feeding difficulties in infants and children

Diagnosis Methods

Diagnosis is made by brain CT or MRI showing markedly reduced ventricles (slit-like appearance); however, small ventricles alone do not establish diagnosis and must be evaluated with symptoms. Comparison with previous imaging is important. To determine if the shunt is functioning, full shunt system radiography, shunt tap test, cerebrospinal fluid flow studies, and in selected cases, intracranial pressure monitoring are used.

Treatment Methods

Treatment is planned individually for each patient. In mild and infrequent symptoms, pain management, avoiding triggers, and follow-up may be sufficient; asymptomatic small ventricles typically do not require treatment. If symptoms are frequent and severe, surgery should be considered: the most common solution is placing a programmable valve with externally adjustable pressure. An additional device (anti-siphon device) preventing excessive drainage may be added, the catheter tip may be repositioned, or in selected patients, an endoscopic procedure (third ventriculostomy) may be attempted to reduce shunt dependency.
