What is this Condition?

Solitary Fibrous Tumor (SFT for short) is a very rare tumor originating from the brain membranes (the membrane layers surrounding the brain). It is usually benign and grows slowly; therefore, it can remain silent for a long time without causing any symptoms and may be incidentally discovered during imaging performed for another reason. Because it resembles brain membrane tumors (meningiomas) on imaging, definitive differentiation is usually made by tissue examination after surgery. The most important characteristic of SFT is that it is rich in blood vessels; numerous blood vessels pass through it. This makes the tumor significant because careful planning is required during surgery. Tumors can range in grades from benign to more aggressive, and can rarely recur over time.

Symptoms

Persistent, progressively worsening headache usually localized to the same area

Seizures (epilepsy-like convulsions)

Weakness or sensory loss in the arm or leg depending on tumor location

Blurred vision or visual field narrowing

Personality changes, slowing of thinking and perception

Nausea and vomiting due to increased intracranial pressure in large tumors; rarely hypoglycemia during fasting (sweating, palpitations, tremor)

Diagnosis Methods

The first-line diagnostic tool is contrast-enhanced brain MRI, which shows tumor location, size, and vascularization. Additional imaging may be performed to determine if the tumor is highly vascular. Definitive diagnosis is made through histopathological examination and special staining (immunohistochemistry) of tissue obtained during surgery; this examination distinguishes the tumor from meningeal tumors.

Treatment Methods

The primary treatment is complete surgical removal of the tumor whenever possible; when feasible, the membrane to which the tumor is attached is also removed to reduce recurrence risk. If the tumor is highly vascular, preoperative embolization of feeding vessels may help limit bleeding. When complete removal is not possible or the tumor is of a more aggressive type, radiation therapy (radiotherapy or radiosurgery) may be added. In advanced or recurrent cases, targeted oral medications may be considered.

Surgery: complete removal of the tumor and its attached membrane whenever possible

Preoperative embolization of feeding vessels to limit bleeding

Radiation therapy (radiotherapy or radiosurgery) when complete tumor removal is not possible

Targeted drug therapy at physician's discretion in advanced or recurrent cases
