What is this Condition?

Transient ischemic attack (TIA) is caused by a brief reduction or interruption of blood flow to a region of the brain. Like ischemic stroke, it stems from reduced blood flow rather than bleeding, but unlike stroke it leaves no permanent injury (infarct). It presents with stroke-like symptoms that begin suddenly (for example, weakness in one arm, speech disturbance, temporary vision loss in one eye) and then resolve, usually within a short time. The defining feature is that no permanent brain damage (infarct) develops; in this respect it differs from stroke. However, TIA should not be taken lightly: it is an important warning sign from the body and the risk of actual stroke increases significantly afterward, especially in the first days. For this reason, evaluation must be done without delay even if symptoms have resolved. Early diagnosis and treatment can significantly reduce stroke risk.

Symptoms

Sudden onset weakness or numbness on one side of the body, in the arm or leg

Speech disturbance: difficulty finding words, understanding, or slurred speech

Temporary vision loss in one eye, like a curtain descending

Sudden severe dizziness, loss of balance, difficulty walking

Double vision or difficulty swallowing

Symptoms begin suddenly and typically resolve completely in a short time (usually within an hour)

Diagnosis Methods

TIA is an emergency; evaluation must be performed as soon as possible (within the first 24-48 hours) even if symptoms have resolved. Brain MRI (especially diffusion MRI) is performed to distinguish whether actual brain damage has occurred; CT can be used to exclude hemorrhage. Neck and brain vessel ultrasound and vessel imaging (CT or MR angiography) are obtained to visualize narrowing in the vessels. EKG, rhythm monitoring (Holter), and cardiac ultrasound are performed to assess possible clot originating from the heart. Blood tests evaluate glucose, cholesterol, and coagulation parameters.

Treatment Methods

The goal of treatment is to prevent a stroke that may follow and rapid initiation is important. Most patients receive medications that reduce blood clotting (such as aspirin, sometimes dual therapy briefly). Cholesterol-lowering medications (statins) are added; if hypertension and diabetes are present, they are strictly controlled. If cardiac arrhythmia (atrial fibrillation) is present, anticoagulant (blood thinner) medications may be necessary. If significant narrowing is detected in the neck vessel, surgical cleaning of the vessel or stent placement may be considered; this procedure is usually planned in the early period. Smoking cessation, weight loss, and regular exercise are integral parts of treatment.

Antiplatelet medications (such as aspirin; brief dual therapy in some patients)

Cholesterol-lowering (statin) medications and strict blood pressure-glucose control

Anticoagulant (blood thinner) medication therapy if cardiac arrhythmia is present

Surgical cleaning of carotid artery or stent placement if significant narrowing is present
