What is this Condition?

Wernicke encephalopathy is an acute condition that develops when B1 vitamin (thiamine) levels are severely depleted, causing damage to specific brain regions. It is most commonly seen with prolonged excessive alcohol use; however, it can also develop with malnutrition, gastrointestinal surgery, severe hyperemesis in pregnancy, and certain chronic diseases. The classic triad consists of confusion, ataxia (balance and gait disturbance), and ophthalmoplegia (eye movement disorder); however, not all three symptoms are present together in most patients. Without timely treatment, it can lead to permanent severe memory loss (Korsakoff syndrome) and even death. Therefore, if suspected, high-dose B1 vitamin is given intravenously without delay.

Symptoms

Confusion, disorientation to time-place-person, slowing of attention and thought

Imbalance, staggering gait, difficulty standing (ataxia)

Ocular movement disorders: involuntary eye tremor, diplopia, inability to move eyes laterally

Apathy, lethargy, in severe cases profound somnolence

Numbness and paresthesias in hands and feet, weakness (associated neuropathy)

Hypotension, palpitations, hypothermia

Diagnosis Methods

Diagnosis is largely based on clinical examination and patient history; particularly in patients with alcohol use or malnutrition history, physicians should consider the diagnosis if confusion, imbalance, or any ocular findings are present. Brain MRI can support diagnosis but normal findings do not exclude the disease. Blood tests may be helpful but treatment is started without awaiting results, as delay can lead to permanent damage.

Treatment Methods

The foundation of treatment is immediate high-dose intravenous B1 vitamin (thiamine); thiamine is a very safe vitamin. As symptoms improve, the dose is gradually reduced and oral maintenance therapy can be started. An important rule: if glucose is to be administered, it must be given together with or after thiamine; giving glucose alone can worsen the condition. Additionally, nutrition is corrected, fluid-electrolyte balance is restored, and physiotherapy is applied for balance disorders.
