What is this Condition?

A brain abscess is a collection of pus (suppuration) within brain tissue caused by microorganisms and eventually surrounded by a capsule. Infection most commonly spreads from adjacent inflammatory foci (sinusitis, otitis media, dental infection), is hematogenously seeded from a distant source, or develops following head trauma or surgery. The risk is higher in immunocompromised individuals. This expanding mass causes symptoms such as headache, fever, weakness in the limbs, and seizures through both inflammatory and mass effect mechanisms. Diagnosis is made by contrast-enhanced brain MRI. Treatment consists of prolonged antibiotic therapy combined, in most cases, with surgical drainage of the pus.

Symptoms

Progressively worsening, severe headache

Fever (may not always be very high)

Focal neurological symptoms such as weakness on one side of the body, speech disturbance, or vision problems

Seizures

Alterations in consciousness: drowsiness, mental confusion, disorientation

Nausea and vomiting, neck stiffness

Diagnosis Methods

The most valuable diagnostic method is contrast-enhanced brain MRI; on this imaging, an abscess appears as a lesion with a ring-like enhancement at its periphery and fluid content inside. Blood tests investigate inflammatory markers and the source of infection. When possible, microbiological examination of the aspirated pus sample identifies the responsible microorganism and guides antibiotic selection. Sinuses, ears, and teeth may also be evaluated to identify the source of infection.

Treatment Methods

Treatment typically involves two concurrent approaches: prolonged (usually 6-8 weeks) intravenous antibiotic therapy and surgical drainage of the pus. When an abscess reaches a certain maturity or becomes large enough to cause mass effect, closed-method needle aspiration (stereotactic aspiration) or, in some cases, surgical evacuation is preferred. In very early stages with small abscesses, monitoring with antibiotics alone may be considered. The source of infection (for example, sinusitis) is also treated separately; when necessary, supportive therapy for seizures and cerebral edema is provided.

Prolonged intravenous antibiotics: Treatment targeting the responsible microorganisms, typically involving multiple agents administered concurrently and lasting several weeks.

Closed-method drainage of pus (stereotactic aspiration): The abscess is aspirated using a fine needle under image guidance; the obtained sample is examined.

Surgical evacuation: In abscesses with thickened capsules, recurrent abscesses, or those in suitable locations, surgical removal may be considered.

Management of source and supportive therapy: The source such as sinusitis, ear, or dental infection is treated; medication for seizures and cerebral edema is given when necessary.
