What is this Condition?

Lumbar disk herniation, commonly known as a lumbar hernia, occurs when the disk between the lumbar vertebrae, which normally acts as a cushion, protrudes outward and compresses the nearby nerve root. The most common symptoms are lower back pain, pain radiating from the buttocks to the leg, numbness in the leg, and weakness. The disk normally functions as a shock-absorbing buffer between the vertebrae; due to degeneration, sudden strain, or heavy lifting, its outer layer weakens and the inner material can protrude outward, causing pain when it contacts the nerve. The good news is that many patients improve without surgery through medication, rest modification, and physical therapy. If pain persists, weakness progresses, or emergency signs affecting bowel and bladder control develop, surgical options are considered; in these cases, the goal is to reduce compression of the nerve. The severity and duration of symptoms vary from person to person; therefore, the treatment plan is individualized for each patient.

Diagnosis Methods

Diagnosis primarily involves a detailed physical examination and magnetic resonance imaging (MRI) of the lumbar spine. During examination, findings indicating whether the nerve is affected are evaluated; if necessary, a nerve test called EMG (electromyography) is performed to assess the degree of nerve involvement. Imaging is correlated with clinical findings to determine at what level the herniation is located and how much it compresses the nerve. This helps clarify whether the symptoms are truly caused by the herniation.

Treatment Methods

The initial approach typically involves pain control, activity modification, and physical therapy. Many patients experience significant improvement with this approach. If symptoms persist, leg weakness progresses, or emergency signs affecting bowel and bladder control develop, surgical options are considered. The goal of surgery is to reduce the compression of the herniated disk material on the nerve. Early mobilization after surgery and an exercise program guided by the physician can positively influence long-term outcomes. Treatment decisions are always made by evaluating the patient's symptoms, physical examination findings, and imaging results together.
