Suboccipital Puncture and Myelography

CSF collection and contrast imaging through the back of the skull

What is This Treatment?

This procedure involves obtaining a CSF sample via cisternal puncture from the suboccipital (C1-C2) region and/or performing myelography with contrast injection. It is preferred when the lumbar route is unsuitable or for posterior fossa pathologies.

Treatment Process

The patient is positioned sitting or lateral, with neck flexion. Under sterile conditions, a needle is directed through the posterior C1-C2 interval into the cisterna magna. CSF is collected or contrast is injected for imaging.

When is it used?

  • CSF sampling when the lumbar route is contraindicated
  • Evaluation of foramen magnum and posterior fossa lesions
  • Diagnosis of craniocervical junction pathologies
  • Situations requiring downward myelography

Recovery and planning

Close neurological monitoring for several hours is required after the procedure. Respiration and consciousness level are observed.
