What is this Condition?

Chronic traumatic encephalopathy (CTE) is a progressive neurodegenerative brain disease that develops slowly over many years following repeated head impacts and concussions. It is particularly seen in individuals who participate in contact sports with frequent head impacts (American football, boxing, rugby) for prolonged periods and in military personnel exposed to blast injuries. In this disease, an abnormal protein (tau) gradually accumulates in the brain. Symptoms typically emerge years or even decades after the trauma, usually in middle age; they may present as mood and behavioral changes (depression, anger, impulsivity) or as memory and cognitive problems. An important point is that one or two concussions are not expected to cause this disease; risk is associated with numerous repetitive impacts. Since definitive diagnosis can only be made through brain examination after death, the diagnosis during life is referred to as "probable" or "suspected."

Symptoms

Depression and significant mood changes

Impulsivity, difficulty controlling anger, and behavioral changes

Memory weakness and difficulty learning new information

Difficulty with planning, organization, and attention-demanding tasks

Cognitive decline in advanced stages (dementia)

Motor slowing, tremor, or balance problems in advanced stages

Diagnosis Methods

Definitive diagnosis can only be established through brain examination after death; during life, the diagnosis is referred to as "possible/suspected." This diagnosis is based on criteria such as a history of repeated head trauma, presence of cognitive and behavioral symptoms, symptom onset years after trauma, and progressive course over time. Brain MRI is not specific to the disease but may show supporting findings such as atrophy in the frontal and lateral brain regions. Specialized imaging methods demonstrating tau protein are still in the research phase. Cognitive function tests and psychiatric evaluation are used to document symptoms and distinguish the condition from similar diseases (such as Alzheimer's disease and depression).

Treatment Methods

There is currently no treatment that halts or reverses the progression of this disease; treatment aims to alleviate symptoms and support quality of life. The most effective approach is prevention: reducing repeated head impacts, not returning to sports before complete recovery from concussion, and considering sports modification when necessary. Appropriate medications and psychotherapy for depression and mood disorders; mood-stabilizing medications for impulsivity and anger can be used. For memory and cognitive problems, certain medications and cognitive training may be attempted. Regular exercise, balanced nutrition, mental activity, and avoidance of alcohol and substance use are also supportive. Close monitoring for suicide risk is important.
