What is this Condition?

Diffuse axonal injury (DAI) is a severe brain injury that typically occurs during rapid acceleration-deceleration movements such as high-speed motor vehicle accidents or falls from height, causing widespread tearing and damage to nerve fibers (connections providing communication between brain regions) in the brain's depths. It is responsible for a significant portion of severe head injuries and trauma-related prolonged loss of consciousness. Because the damage is usually at the microscopic level and spread across a wide area of the brain, the initial CT scan may appear near-normal; nevertheless, the patient may be in a coma from the moment of trauma. Symptoms typically begin from the moment of trauma and present as prolonged loss of consciousness. The course is highly variable and depends on the severity of damage, patient age, and duration of consciousness loss. There is no specific medication for this condition; treatment is based on intensive care support and long-term rehabilitation.

Symptoms

Prolonged loss of consciousness (coma) beginning at the moment of trauma

No response or very limited response to stimuli

Significant attention, memory, and thinking difficulties upon emergence from prolonged coma

Inability to remember events around the time of trauma

Behavioral changes (restlessness, impulsivity)

Movement disorders in arms and legs, muscle rigidity, or balance problems

Diagnosis Methods

Brain CT scans obtained in the acute phase often appear near-normal or show only diffuse swelling; this is because most of the damage is at the microscopic level. Therefore, a normal CT does not exclude this injury; a coma presentation from the onset following high-speed trauma suggests this condition. MRI imaging is more valuable for definitive evaluation; special MRI techniques can demonstrate damage to deep brain fibers and small hemorrhagic foci. Consciousness level is monitored using standard scales; during recovery, tests measuring attention, memory, and thinking functions are used for rehabilitation planning.

Treatment Methods

Since there is no specific medication for this injury that reverses the damage, treatment is based on intensive care support and management of complications. Patients with profound loss of consciousness require respiratory support (intubation and mechanical ventilation). Intracranial pressure can increase and is monitored; when necessary, measures such as brain edema-reducing medications, sedation, and proper positioning are implemented. If there is risk of seizures due to additional brain injury, seizure prophylaxis may be started in the initial period. Once the medical condition stabilizes, early and intensive rehabilitation (physical therapy, occupational therapy, speech therapy, and cognitive training) is of great importance. Recovery is slow and can take months to years.
