What is this Condition?

Scalp defect refers to loss or damage in the hair-bearing skin and underlying soft tissue layers covering the skull. This region can be damaged due to accidents, blunt trauma, sharp injuries, tumor removal, infection, burns, or healing problems from previous surgeries. The scalp has extremely rich blood circulation, so when injured it can bleed profusely; however, this same rich circulation also facilitates healing. If the exposed area is small, it may close spontaneously; for larger or deeper losses, repair methods such as sutures, skin grafts, tissue flaps (shifting surrounding tissue), or tissue expanders may be needed. The goal is both to protect the skull and to provide as natural an appearance as possible.

Symptoms

Open wound and tissue loss in the scalp; sometimes the underlying bone may be visible.

Heavy bleeding from the wound; bleeding can be prominent due to the scalp's rich vascular structure.

Pain, tenderness, and swelling around the wound area.

Non-healing or very slowly healing wound with discharge or foul odor (may indicate infection).

Permanent hair loss (baldness) in the wound area and scar formation.

Skin thinning in areas of previous surgery or radiotherapy, easy reopening, or chronic wound.

Diagnosis Methods

Diagnosis is primarily made through physical examination: the physician evaluates the size and depth of the defect, which tissue layers are affected, and the viability of wound margins. When bone or deeper structures may be involved, CT imaging shows skull fractures and bone loss, while MR imaging shows soft tissue and deeper involvement. If infection is suspected, blood tests and wound culture may be taken; the blood circulation of the area is also examined for repair planning.

Treatment Methods

Treatment is planned according to the size and depth of the wound and generally follows a stepwise approach from simple to complex. In the initial phase, bleeding control, wound cleaning, and if necessary, tetanus and antibiotic protection are provided. Small wounds may close with sutures or spontaneously. For larger losses, skin grafts (thin skin taken from another area), flap reconstruction (shifting surrounding healthy tissue to close the wound), or tissue expanders are used. When bone or brain membrane is also affected, these structures are repaired first, then covered with living tissue. The method selection is determined by the physician based on the individual and situation.

Closing the wound with sutures or appropriate care (in small defects).

Skin grafting: placement of a thin layer of skin taken from another body area onto the open area.

Flap repair: shifting nearby healthy hair-bearing skin along with its own blood vessels to close the wound; advantageous in appearance since the hair-bearing area can be preserved.

Tissue expander: gradual expansion of the patient's own hair-bearing skin using a balloon placed under the skin and inflated progressively over weeks for use in repair.
