What is this Condition?

Traumatic subarachnoid hemorrhage (SAH) is bleeding into the thin fluid space surrounding the brain surface (subarachnoid space) following head trauma. It is the most common type of hemorrhage in moderate to severe head injuries. Blood from torn small vessels on the brain surface seeps into this space containing cerebrospinal fluid. Mild cases are those without accompanying brain damage and with an alert consciousness; the majority of these have a good outcome. Cases where hemorrhage is extensive or other brain injuries are present are more serious. This hemorrhage must be distinguished from spontaneous hemorrhage due to rupture of an aneurysm (vascular aneurysm); trauma history and distribution pattern on imaging help in this differentiation. Possible complications include obstruction of cerebrospinal fluid circulation (hydrocephalus) and, more rarely, cerebral vasospasm.

Symptoms

Sudden onset, severe and widespread headache

Neck stiffness and light sensitivity

Nausea and vomiting

Altered consciousness, drowsiness (in more severe cases)

Weakness on one side of the body or changes in eye findings

Seizures

Diagnosis Methods

Brain computed tomography (CT) is the first and most important examination; it directly shows blood accumulated in the sulci and spaces on the brain surface. If trauma history is unclear or if the distribution of hemorrhage suggests aneurysmal rupture, CT angiography can be added to visualize blood vessels. Lumbar puncture (cerebrospinal fluid sampling) is not used for diagnosis in this condition as it carries risks. Control CT can be performed within the first 24-48 hours to determine if hemorrhage is progressing; emergency imaging is performed if consciousness worsens or new findings appear.

Treatment Methods

Most patients with traumatic SAH require only supportive (non-surgical) treatment. Mild cases (alert consciousness, thin hemorrhage, no additional damage) are usually observed closely for 24-48 hours; hourly neurological examination is performed and discharge is possible if the condition is stable. Appropriate analgesics are given for headache. When additional brain damage is present, seizure prophylaxis medications may be started in the initial period. If cerebrospinal fluid circulation is obstructed and hydrocephalus develops, external ventricular drainage can be applied. In severe cases, intracranial pressure monitoring and brain edema-reducing treatments are implemented in the intensive care unit. Treatment is planned individually for each patient.
