What is this Condition?

A dural arteriovenous fistula is an abnormal direct connection between arteries in the dura mater (the tough membrane surrounding the brain) and venous drainage channels or cortical veins that drain blood from the brain. Normally, blood flows from arteries to capillaries and then to veins; in this condition, high-pressure arterial blood bypasses the capillaries (a type of 'short circuit') and flows directly into veins. Most cases are acquired (not congenital) and may develop following vascular occlusion, trauma, surgery, or inflammation. The severity of the condition depends on the direction of blood flow: some types typically cause mild symptoms (such as pulsatile tinnitus in the ear), while others increase venous pressure and carry risk of intracranial hemorrhage, stroke, or seizures. A subtype involving the orbit can cause periocular swelling and redness. Treatment is planned according to type and symptoms; early diagnosis leads to favorable outcomes.

Symptoms

Pulsatile tinnitus synchronous with heartbeat in the ear (most common symptom)

Persistent headache localized to a specific region

In high-risk types, intracranial hemorrhage: sudden severe headache, nausea, altered consciousness

Weakness in an arm/leg, speech disturbance, or seizures

In orbital type: exophthalmos, redness, diplopia, or vision impairment

In spinal type: progressive weakness in legs and urinary/bowel dysfunction

Diagnosis Methods

Initial evaluation uses contrast-enhanced brain MRI and MR venography to visualize draining veins; abnormal vascular structures, venous occlusion, and signs of prior hemorrhage may be seen. CT angiography can also provide detailed vascular imaging. The gold standard for definitive evaluation is catheter-based digital subtraction angiography (DSA); it most accurately shows the exact location of the fistula, feeding arteries, drainage pattern, and relationship to eloquent brain structures. This information is critical for treatment planning.

Treatment Methods

Treatment is planned based on drainage pattern, symptoms, hemorrhage history, and fistula location. In low-risk types without symptoms or with only mild tinnitus, especially in elderly patients with high surgical risk, regular MRI follow-up may be considered. In higher-risk types, endovascular embolization is usually the first choice: specialized materials are delivered through a catheter to the feeding vessels, closing the abnormal connection. For fistulas not accessible endovascularly or those that are complex, open surgery (craniotomy) can be used to close the fistula. In selected cases, focused radiation therapy (radiosurgery such as Gamma Knife) may be an option; its effect develops over months to years. Treatment is individualized for each patient.
